Laparoscopic Roux-en-Y Gastric Bypass

Abstract
Gastric bypass was first described by Mason and Ito in 1966. At that time, it was a novel procedure, which potentially could replace the jejunoileal bypass, a malabsorptive procedure associated with many serious complications. First, it was performed in a loop fashion and afterwards, in a Roux-en-Y fashion, as described by Griffen in 1977. Today, Roux-en-Y gastric bypass is considered the ¡°gold standard¡± of bariatric treatment in US and about 51% of the bariatric procedures done in US/Canada is this mentioned procedure. Many bariatric procedures are now discarded as bariatric operation, such as the jejunoileal bypass and the vertical banded gastroplasty; however, the gastric bypass stood the test of time and now is the most popular bariatric procedure worldwide. Not only is the weight loss effect well known after Roux-en-Y gastric bypass, but also the metabolic effect of this procedure. Recently, two randomized controlled trials comparing intensive medical therapy vs. Roux-en-Y gastric bypass showed the superiority of this procedure for the resolution of type II diabetes. It is reasonable to expect more important role of this procedure in the field of metabolic surgery.
